Participation Waiver & Release of Liability

Ignite Youth Sports LLC (“Ignite”) provides non-contact youth sports instruction and recreational multi-sport activities. These activities may include, but are not limited to, soccer, flag football, basketball, agility training, fitness games, and general physical exercise.

While Ignite programs are designed to be safe and age-appropriate, all physical activity carries inherent risks. These risks may include slips, trips, falls, collisions, overexertion, injuries related to field or weather conditions, and other unforeseen hazards.

Assumption of Risk

I understand and acknowledge that participation in physical activity may result in injury, including but not limited to sprains, strains, fractures, concussions, dehydration, or other bodily injury. I knowingly and voluntarily assume all risks, both known and unknown, associated with my child’s participation in Ignite programs.

Release of Liability

In consideration of my child’s participation in Ignite programs, I hereby release, waive, and discharge Ignite Youth Sports LLC; its owners, directors, coaches, employees, contractors, volunteers, agents, facility partners, and any associated school or district from any and all claims, liabilities, demands, or causes of action arising out of or related to participation in Ignite programs, except in cases of gross negligence or intentional misconduct.

Hold Harmless & Indemnification

I agree to indemnify and hold harmless Ignite Youth Sports LLC from any claims brought on behalf of my child or arising from my child’s participation or conduct during the program.

Medical Authorization

I authorize Ignite staff to administer basic first aid and to seek emergency medical treatment for my child if deemed necessary. I understand that I am financially responsible for any medical care or expenses resulting from participation.

Participant Conduct

I understand that participants are expected to follow all safety rules and coach instructions. Disruptive, unsafe, or aggressive behavior may result in removal from the program without refund.

Media Release (Optional)

Ignite Youth Sports may take photographs or video during program activities to highlight our community, celebrate participation, and share program moments. Granting permission for your child’s image to be used for promotional or social media purposes is completely optional and may be selected during registration.

Refund Policy

I understand that Ignite Youth Sports does not offer refunds for missed sessions, scheduling conflicts, or voluntary withdrawal from the program.

Acknowledgment

By completing registration and selecting the required waiver acknowledgment during checkout, I confirm that:

• I have read and understand this Participation Waiver & Release of Liability

• I agree to be legally bound by its terms

• I am the parent or legal guardian of the minor participant

• I voluntarily consent to my child’s participation in Ignite Youth Sports programs
